Waiver, Release of Liability and Consent to Medical Attention
In exchange for my being allowed to participate in the Cutting Edge Fitness (CEF) program (the “Activity”), I and if I am not yet 18 years of age, my parent or legal guardian (individually and collectively referred to below in the first person singular) agree to be bound by each of the following:
Identification of Risks – I understand that participation in the Activity my involve risk of injury, disability.
Assumption of Risk – I am physically and psychologically ready to participate in the Activity and assume all risks connected with my participation in the Activity. I accept personal responsibility for any liability, injury, loss or damage in any way connected with my participating in the activity.
Waiver and Release – I release and discharge CEF and each of their employees, from all claims for any liability, injury, loss or damage in any way connected with my participation in the Activity, whether or not caused in whole or part by the negligence of any of the organizations or individuals mentioned above. I intend for this waiver and release to also apply to my relatives that might pursue any legal action or claims for such liability, injury, loss or damage.
I further intend that this waiver and release shall be effective indefinitely, including participation in other programs or opportunities of CEF.
Consent to Medical Treatment – I agree that CEF (including its employees) may, but has no duty to provide medical treatment
Provide me, through medical personnel of their choice, customary medical or training assistance, transportation, and emergency medical services.
I have read this Waiver, Release and Consent form and understand that I have given up substantial rights by signing it. I am signing this Waiver, Release and Consent form voluntarily.
If the person(s) participating in the Activity, is not yet 18 years old: As a parent or legal guardian of the child (children) named on the Membership Application, I verify that I fully agree to, understand, and accept all provisions of this Waiver, Release and Consent form.
Philosophy: To enhance athletic performance & conditioning levels in speed/agility. This IS NOT a conditioning camp however, every movement will be done at 100% intensity. The highest quality of coaching & instruction will be given to you; correcting & improving your running form. We want to give you the understanding of how the body works biomechanically & the confidence to correct your own running form. 40 yard dash, Pro shuttle & L-Cone Drill will be the main emphasis in improving your times.
Collegiate athletes looking to improve in the testing: 40 yard dash, pro shuttle, or L-Cone Drill. Athlete’s looking to improve speed & agility for their respective sports/season. It does not matter what sport you play or what shape you are in; this camp will enhance your overall athleticism & give you the confidence to compete at the highest level of competition.
What to Bring:
*Proper footwear (Cross-Trainer works best)
*Water Bottle 30-40 oz. (be sure to drink plenty throughout the day-100oz)
*Clothing- loose, breathable, light colored
*Eat 2 hours prior to camp. Bring something to eat as well (NO CANDY OR SODA!) A snack with Protein & Carbohydrates is preferable.

Jason Laube has a Master’s of Education in Exercise Physiology & Nutrition from the University of Minnesota & brings 11 years of coaching experience. Jason has worked with some of the best athletes in the NFL (Kansas City Chiefs); NFL Europe (Amsterdam Admirals: World Bowl XIII Champions); University of Minnesota Golden Gophers – Assistant Director of Strength & Conditioning/Football; 2 WNBA Basketball Players; Olympic Athletes/NFL Athletes (Velocity Sports Performance – Missouri City, Texas), & 12 Varsity sports at UW-River Falls. Jason has also coached two-years at the University of UW-River Falls – Assistant Outside Linebacker/Strong Safety Coach. Jason is owner & Sports Performance Director at Cutting Edge Fitness.
